What this role involves

The post sits within the Hounslow Mental Health Integrated Network Team (MINT), an integrated community and outpatient service that accepts referrals across the age range, from young people transitioning out of CAMHS through to specialist older adults. Working Monday to Friday in secondary care, the post holder helps deliver integrated, personalised, place-based care under the Community Mental Health Framework for adults.

• Autonomously manage a caseload of routine to complex cases, keeping care plans and risk assessments up to date.
• Screen service users for physical and mental health needs, including weight management, diabetes risk and smoking cessation, and provide related education.
• Support the running of the local depot clinic and physical health clinics, including stock management and administration of depot medication.
• Advise service users and their wider “triangle of care” on medication, side effects and contraindications.
• Liaise with GPs, primary care networks, inpatient wards, crisis teams, social care and housing providers regarding ongoing needs.
• Support transfers between services or discharge back to the GP at the end of treatment.
• Escalate concerns through the wider MDT using local processes and contribute to local service improvement work.
